All Hormone & Wellness Memberships include supplies for weekly injections. All four tiers require a three-month minimum. Initial labs available at an additional cost.

Disclaimer: Getting labs drawn or refilling a medication automatically re-enrolls your membership for another three months.
School age to 17*: $45/month (requires an accompanying parent membership)
Ages 18–39: $135/month
Ages 40–64: $155/month
Ages 65 and older: $175/month
Minimum commitment: twelve (12) months from the enrollment date.
A 10% discount is available to members who choose to pay the annual fee in full at enrollment.
Membership includes unlimited messaging with the care team for medically appropriate concerns. Quick phone calls and same-day visit requests are initially triaged by clinical staff rather than routed directly to the provider.
Laboratory testing is billed separately and is not included in the membership fee.
Members on Medicare must complete a Medicare opt-out agreement as a condition of enrollment, consistent with applicable Medicare regulations.
A separate controlled-substance patient agreement is required as part of enrollment for any member being treated with a controlled substance.
