Governing law. These Terms are governed by the laws of the State of Georgia, and any dispute will be resolved in the state or federal courts located in Forsyth County, Georgia.

Termination. We may suspend or end your access to the Site at any time, for any reason, without notice. Provisions that by their nature should survive termination, such as intellectual property rights and limitations on liability, will continue to apply.

Electronic agreement. Using the Site to agree to these Terms, or to any electronic disclosure we provide, counts as your signature and binds you the same as if you'd signed on paper.

We can't respond to every comment, and nothing posted on our Social Media Sites should be treated as medical advice, diagnosis, or treatment. For questions about your own care, contact your provider directly. If you're experiencing a medical emergency, call 911.

Opinions and views expressed on our Social Media Sites belong to the individuals who post them, not to Kith & Kin. We don't actively moderate every comment, but we reserve the right to review, edit, or remove content that's inappropriate, abusive, off-topic, or spam, and to restrict, suspend, or block access to our Social Media Sites at any time, for any reason.

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This notice describes the privacy practices of Kith & Kin Medical ("Kith & Kin," "we," "us," or "our") and applies to all providers and staff who see you here. For purposes of this notice, "we," "us," and "our" refers to Kith & Kin.

We understand that medical information about you is personal, and we're committed to protecting it. We create a record of the care and services you receive at Kith & Kin. We need this record to provide you with quality care and to comply with legal requirements. This notice applies to all records of your care generated at Kith & Kin.

This notice describes how we may use and disclose your medical information, and your rights regarding that information. We're required by law to keep information that identifies you private, to give you this notice of our privacy practices, and to follow the terms of the notice currently in effect.

For Treatment. We may use and share your medical information to provide your care. For example, if you see more than one provider at Kith & Kin, they may share information with each other to coordinate your treatment, such as your history, the products or procedures you've had, and any medications or allergies relevant to a planned service.

For Payment. We may use and disclose information about your treatment to bill and collect payment from you or, where applicable, your insurance company or a third-party payer.

For Health Care Operations. We may use and disclose your information for our own operations, such as quality assurance, staff training and evaluation, and business planning. We may remove information that identifies you before using it this way whenever possible.

Appointment Reminders. We may use your contact information to send appointment reminders, by phone, mail, text, or email if you've agreed to that method. A reminder may include your name and the date, time, and location of your appointment.

Treatment Alternatives and Related Services. We may use your information to tell you about treatment options or services that may be of interest to you.

Individuals Involved in Your Care or Payment. With your agreement (or when it's clearly in your interest and you haven't objected), we may share relevant information with a family member or other person involved in your care or who helps pay for it.

Business Associates. Some services are provided on our behalf by outside vendors, such as our practice management or billing platforms. We require these "business associates" to appropriately safeguard your information.

Psychotherapy Notes. Any psychotherapy notes we hold will only be used or disclosed with your written authorization.

Research. We generally get your written authorization before using your information for research, except in limited circumstances approved by an Institutional Review Board or Privacy Board.

Other Uses and Disclosures. Any use or disclosure not described in this notice will be made only with your written authorization, which you may revoke in writing at any time (this won't undo disclosures we've already made in reliance on it).

Right to Inspect and Copy. You may ask to inspect and copy the medical information used to make decisions about your care, in writing, at the contact below. We may charge a reasonable fee for copies. In limited circumstances we may deny a request, and you may ask that the denial be reviewed.

Right to Amend. If you believe information we have is incorrect or incomplete, you may ask us to amend it, in writing, with a reason supporting the request. We may deny the request in certain circumstances, such as if the information is accurate and complete.

Right to an Accounting of Disclosures. You may request a list of certain disclosures we've made of your information, going back up to six years. The first list in a 12-month period is free; we may charge for additional lists.

Right to Request Restrictions. You may ask us to limit how we use or disclose your information for treatment, payment, or operations, and you have the right to request that we not disclose information to a health plan for services you've paid for in full out of pocket. We're not required to agree to most restriction requests, but we will comply with a request that meets the out-of-pocket condition above.

Right to Request Confidential Communications. You may ask us to contact you about medical matters in a specific way or at a specific location (for example, by mail only). We'll accommodate reasonable requests without asking why.

Right to Be Notified of a Breach. You have the right to be notified if there's a breach of your unsecured health information.

Right to a Paper Copy. You may request a paper copy of this notice at any time, even if you've agreed to receive it electronically.

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer (contact below) or with the Secretary of the U.S. Department of Health and Human Services. You will not be penalized for filing a complaint.

We reserve the right to change this notice. A revised notice will apply to medical information we already have as well as information we receive in the future. The current notice will be posted at our office and on our website, and you may request a copy at any time.
